Santa Ana, Calif. (August 26, 2015) – South Coast Post Acute, Southern California’s leading post-acute care facility has one of the lowest rehospitalization rates in California and the United States based on recent clinical data.
Rehospitalization rates are measured for patients who are discharged from a post-acute community only to be readmitted to a medical facility within 30 days of the discharge date. According to recent data released by Medicare, 23.5% of all Medicare patients nationwide are readmitted into a medical facility within 30 days of discharge from a post-acute community.
The California State Average rehospitalization rate is 19.2% which is identical to the Orange County Average of 19.2%. Compare that to South Coast Post Acute’s rehospitalization rate of only 7.2%. We send more patients homes and they stay there. These number reflect clinical data between July 1, 2014 and July 1, 2015.
