Post Acute

Maximize Patient Outcomes with Post Acute Rehabilitation

Post Acute rehabilitation is defined as a temporary stopping place on a patient’s journey home after having major surgery. While a hospital’s main purpose is to stabilize patients and begin the recovery process, for some patients, a quick recovery is unrealistic. Sometimes, patients require continued recovery and post-acute care. At South Coast Post Acute, we continually care for our patients after their hospitalization in an effort to maximize their outcomes.

What You Can Expect in Post Acute Care

To achieve the goal of getting healthy, patients receive physical, occupational, and speech therapy from highly skilled therapists. Each team member brings a unique set of skills to the recovery process including:

  • Nurses:  Both registered nurses and nursing assistants provide basic hygiene for patients, and administer medication.
  • Occupational Therapists: This type of therapy focuses on daily activities like brushing your teeth and feeding yourself. Additionally, occupational therapists modify basic tools like forks and knives to allow patients with disabilities to be able to use them.
  • Physical Therapists: Their goal is to promote mobility and relieve pain through exercise and other therapy.
  • Speech and Language Pathologists: These specialists work with patients who have trouble swallowing and help with their speech due to a stroke.

The specialized team, along with the patient and their family, work together to develop an individualized plan of care and help patients get back to living their lives.

Developing A Plan to Get You Home

The whole purpose of post-acute rehabilitation is to get patients well enough to be able to transition back home. Patients’ treatment plans and recovery goals are discussed and developed from the very beginning when you collaborate with your physician. Your care team will create an individualized plan that’s tailored to your needs, and we will communicate with you every step of the way as you progress toward your unique goals.
